Please download the two page form, fill out, sign and bring with you on your first session, email or drop it in the mail to the address on page two of the form.
To Print:
1. Click on form and drag to your desktop.
2. Open image from your desktop and print from your computer and bring with you to your first class.
Send via Email:
1. Click on image and drag to your desktop.
2. Import file into Word or desktop publishing software, fill and sign, save to your desktop.
3. Email back to us and attach the file to the email.
